What I was told to do is:
Send email to bonus@365bonitas.com with code BANKROLL365 in subject line; include your info, etc.

However, I wouldn't hold your breath because I sent THREE e-mails on the Sept.4th, 10th and 12th and still have not gotten a reply from those idiots. Live Chat is "not available" 24 hours a day... I suppose it's possible that the support staff is EXTREMELY busy; OR they have the absolute worst support with which I have ever had the privilege of not dealing.
